Your PCP may need additional information for your care. You may be referred for:
Before you have any outside service, you should make sure that it is covered by your insurance. If they do not cover the service you will be responsible for payment.
The results of the tests will be sent to your PCP here at ICHC.
ICHC will send you a letter or phone you about the test results. When we write or phone we will let you know if you need to schedule a follow-up appointment with your PCP.
If we have not given you the test results after 2 weeks, please call your PCP at 676-6177.
Your PCP may refer you to a specialty provider. You are responsible for payment to the specialist.
If you have insurance, your insurance company may require you to get approval before they will pay for the specialist. There may also be a 6 to 9 month waiting period before the insurance will cover a pre-existing condition. ICHC referral staff can contact your insurance company for you and help you with this.
If you make an appointment to a specialist on your own, without a written order from your PCP, the service may not be covered by your insurance. You are responsible for payment to the specialist office for any charges.
