Primary Care Team Approach
ICHC’s care team coordinates your primary health care needs. Our clinical staff includes:
- family practice, pediatric and internal medicine physicians
- family practice nurse practitioners
- physician assistants-certified
- psychiatrists
- mental health specialists
- nurses
- medical assistants
- case managers
We assign each patient to a Primary Care Provider, or PCP. We try to schedule all of your medical appointments with your PCP.
- Your PCP may be a doctor, nurse practitioner or physician assistant.
- Your PCP will help you with referrals to specialists or other providers, if needed.
- Your PCP will make referrals and coordinate behavioral health services with the ICHC mental health professionals.

Well Child Care Appointments
Getting your child regular health screenings and immunizations is very important. The American Academy of Pediatrics recommends the following ages for well-child appointments.
- 3-5 days (new born)
- By 1 month
- 2 months
- 4 months
- 6 months
- 9 months
- 12 months
- 15 months
- 18 months
- 24 months (2 years)
- 30 months (2 ½ years)
- 3 years
- 4 years
Children should be seen every year ages 5 – 21.
Adult Preventive Care
The three most important things that adults can do to prevent disease and be healthy:
- Be tobacco free
- Be physically active
- Eat a healthy diet
Adults should get the right kinds of health services – screenings, counseling, and preventive medicines – at the right times. Check with your PCP about what you need for the following:
- Heart health (blood pressure, cholesterol, diet)
- Cancer Screenings (breast, cervical, colorectal)
- Health Risks (tobacco use, obesity, alcohol misuse)
- Sexual Health
- Immunizations
