How to Pay for Your Care

If You Have Insurance

Bring your insurance card and/or coupon to every visit. ICHC will bill most insurance plans, including:

  • DSHS medical coupons
  • Basic Health through Community Health Plan or Molina
  • Healthy Options through Community Health Plan or Molina
  • Medicare
  • Medicaid
  • First Choice
  • Premera Blue Cross (in-state and out-of-state)
  • Regence Blue Shield
  • Tricare
  • Labor and Industry claims

If you have a co-pay, it must be paid at the time of your appointment. You will be billed if your insurance does not pay all or part of the charges. If you have any questions about which plans we can accept, please call our Customer Service Representative at 676-6177, ext. 3175.

The Basic Health Plan and Healthy Options

  • Make sure that Interfaith Community Health Center is listed as your Primary Care Provider (PCP). If you do not know who your primary care provider is, call your health plan:
    Community Health Plan: 1-800-440-1561
    Molina: 1-800-869-7165
  • If you have Healthy Options you must be assigned to Community Health Plan or Molina. If you need to switch to one of these plans, call (800) 562-3022 for assistance.

If You Do Not Have Insurance

A minimum payment is due at the time of service. You will be billed for any fees over the minimum amount due.

We can help you apply for health insurance. On-site DSHS staff can help you apply for Medicaid. Our Customer Service Representative can help you apply for Basic Health Insurance. For more information contact ICHC Customer Service at (360) 676-6177.

Sliding Fee Scale Discount

ICHC has a sliding discount for patients who have no insurance and are not eligible for Medicaid. The discounts are based on household income. You need to show us proof of income in order to qualify for the discount.

Bring any information you have on the following types of income that you receive:

  • Pay check stubs—bring your last two
  • Last year’s 1040 or 1040A income tax return
  • Unemployment compensation
  • Social Security benefit information
  • Disability benefits
  • Worker’s compensation
  • Veteran’s benefits
  • Alimony
  • Child support
  • Pension
  • Interest
  • Dividends
  • Rental Income
  • Estate/Trusts
  • Other sources of income
