Eligibility: First of the month following date of hire for regular full-time & part-time employees working 20 hours per week or more.
Summary: ICHC provides and pays for full coverage for our base plan for regular, full-time employees and pays a pro-rated premium for regular, part-time employees (working 20 hours or more per week) based on their full-time equivalent (FTE) rounded to the nearest tenth. This base plan consists of a High Deductible Health Plan (HDHP) combined with a Health Savings Account (HSA) into which ICHC deposits a portion of the funds required to meet the deductible. A buy-up option is available for which the employee pays the cost difference between the base plan & the buy-up. Employee may elect to cover eligible dependents through pre-tax payroll deduction at 100% of cost. Individual dependent plans are available from our broker that are subject to underwriting but may be a cheaper alternative (premiums are not pre-tax for these plans, however). Provider: Direct Reimbursement Program through ICHC.
Eligibility: First of the month following date of hire for regular full-time & part-time employees working 20 hours per week or more.
Summary: ICHC provides and pays full coverage for the standard plan for regular, full-time employees and pays a pro-rated premium for regular, part-time employees (working 20 or more hours per week) based on their full-time equivalent (FTE) rounded to the nearest tenth. Employee may elect to cover eligible dependents through a pre-tax payroll deduction. When an employee goes to the dentist or vision provider to receive care, they must first pay for the visit then submit proof of payment to ICHC for reimbursement of 80% of their first $1,250 of eligible dental expenses (a $1,000 benefit) & 80% of their first $300 of eligible vision expenses (a $240 benefit). Eligibility: First of the month following date of hire for all employees.
Summary: This plan allows you to place pre-tax dollars into a 401(k) retirement account. Employees may begin contributing the first of the month following their hire date, and can contribute 1 – 100% of their eligible earnings not to exceed $17,000 in the 2012 plan year, or $22,500 if you are age 50 or older. ICHC will match employees’ first 4% of contributions after one year of employment. All funds are immediately 100% vested. Plan details are located in 401(k) booklet & Summary Plan Description.

Eligibility: First of the month following date of hire for regular full-time & part-time employees working 20 hours per week or more.
Summary: ICHC provides $25,000 of Life/AD&D for all eligible employees.

Eligibility: You may apply for up to five times your salary in $10,000 increments, to a maximum of $250,000, within 31 days of the first of the month following hire date for regular full-time & part-time employees working 20 hours per week or more. If you apply within 31 days, you are eligible to purchase up to $130,000 of coverage without answering any medical questions or providing evidence of insurability.
Summary: Coverage for spouse & children is also available for purchase. See Voluntary Life/AD&D packet for information and pricing. This would be via payroll deduction.
Provider: Assurant.
Eligibility: You may apply for coverage without answering any medical questions or providing evidence of insurability within 31 days of the first of the month following hire date for regular full-time & part-time employees working 20 hours per week or more.
Summary: This Short Term Disability insurance would pay the amount you select, from $100 up to $1000, not to exceed 60% of your basic covered weekly pay, for up to 11 weeks, after an Elimination Period of 14 days, if you meet the definition of disability. See Voluntary Short Term Disability Packet for information and pricing. This would be purchased via payroll deduction.

Eligibility: You may apply for coverage without answering any medical questions or providing evidence of insurability within 31 days of the first of the month following hire date for regular full-time & part-time employees working 20 hours per week or more.
Summary: This Long Term Disability insurance would pay the amount you select, from $500 up to $5000, or 60% of your basic covered monthly pay, after a 3-month qualifying period, up to the later of age 65 or 24 months, if you meet the definition of long-term disability. See Voluntary Long Term Disability Packet for information and pricing. This would be purchased via payroll deduction.

Eligibility: You may apply for coverage without answering any medical questions or providing evidence of insurability* within 31 days of the first of the month following hire date for regular full-time & part-time employees working 20 hours per week or more. If you apply after this, your coverage will be subject to medical underwriting. Eligible family members may also apply for coverage under our group plan, but they are always subject to underwriting & may have to pay via bank draft.
Summary: There are four LTC plans to choose from, each providing a monthly benefit between $2,000 to $9,000 per month with Benefit Durations of 3-year, 6-year, & Lifetime. Information & pricing are located in Voluntary Long Term Care packet. This would be purchased via payroll deduction.
*Over $6,000 monthly benefit &/or Lifetime duration are always subject to medical underwriting.
Provider: Allegiance Benefit Plan Management, Inc.
Eligibility: First of the month following date of hire for regular full-time & part-time employees working 20 hours per week or more.
Summary: This IRS program allows employees to set aside pre-tax dollars to be used for 1) medical/dental benefit premiums (automatic enrollment); 2) out-of-pocket medical expenses (Medical Spending Account), & 3) dependent care expenses (Day Care Account). Details of the plan are located in the Flexible Spending Account packet. NOTE: Employees who enroll in the Group Health High Deductible Health Plan with the Health Savings Account are not eligible for the FSA Medical Spending Account (Item 2 above).
Provider: Health Promotion Network (HPN) through St. Joseph Hospital / Peace Health.
Eligibility: First of the month following date of hire for regular full-time & part-time employees working 20 hours per week or more.
Summary: The EAP is a program that provides confidential, professional counseling and assistance to employees and their dependents who are experiencing personal or work-related problems. Employees & their dependents may receive up to three (3) free counseling sessions per incident. Details are located in EAP brochure.
ICHC offers a generous paid time off bank of 20 days per year for the first year of employment and 26 days per year starting the second year of employement to all full-time employees. Paid Time Off (PTO) leave combines what would traditionally be called vacation and sick leave into one paid time off bank. Part-time employees working 20 hours per week or more will receive a pro-rated earned leave bank.
Regular full-time employees receive six (6) days of holiday pay per year. Part-time employees working 20 hours per week or more receive pro-rated holiday pay. Please refer to the Personnel Policies and Procedures Manual for more details and a list of holidays.
In addition to the paid time off provided above, ICHC also offers a variety of other leave programs as described in our Personnel Policies & Procedures Manual.
